Introduction

Booking and Cancellation Policy

This policy describes your financial and scheduling responsibilities.

 

Consultation Fees

  • Surgical Phone Consultation with Surgical Coordinator: Complementary
  • Surgical In-Person Consultation with Surgeon: $100
  • Surgical In-Person Re-Consult with Surgeon: $100
  • MedSpa Consultation WITHOUT same day service/treatment: $50 (fee not charged until consult)
  • MedSpa Consultation WITH a same day service/treatment: Complementary

Cancellations & No-Shows

MedSpa Consultations and Services:

  • Appointments canceled with at least 48 hours’ notice are refundable.
  • A $50 fee will be charged to the card on file for no-shows or cancellations made within 48 hours
    of the scheduled appointment.

Surgical Consultations:

  • The $100 consultation scheduling fee is refundable if the appointment is canceled at least 48
    hours in advance.
  • The $100 consultation scheduling fee is non-refundable for no-shows or cancellations made
    within 48 hours of the appointment.
  • Failure to CONFIRM the in-person surgical consultation at least 48 hours prior to the
    appointment will result in automatic forfeiture of the appointment and is non-refundable.

 

Online Booking

Please Note:

  • All online bookings are appointment requests only. A team member will review and confirm your
    request by the next business day.
  • A valid credit or debit card is required to reserve your appointment.
  • New and existing patients: For secure processing, please call (262) 269-1050 to add a card on
    file. Your card will not be charged until the time of the appointment.
  • Appointment requests will not be confirmed until a card is on file.

Medspa Services & Products

  • Payment is due at the time of service.
  • All products are final sale — non-returnable and non-refundable.

 

Late Arrival / Incomplete Documentation

  • Arriving more than 10 minutes late or failing to complete required documentation prior to your
    appointment may result in rescheduling, cancellation, or a shortened treatment.

Treatment Expiration Policy

  • All MedSpa treatments or packages must be completed or scheduled within 12 months of the
    redemption date.
  • After 12 months, unused treatments/sessions will expire and no longer be eligible for redemption.
  • Gift cards must be redeemed within 12 months of original purchase date.

Revision Policy

  • All spa or surgical revisions must be completed or scheduled within 12 months of the original treatment
    date.
  • Revisions must be “reasonable” and are at the discretion of the practicing physician
  • After 12 months, any revision is subject to provider discretion and may no longer be available or
    applicable.
  • Revisions may result in additional fees.

Surgical Booking & Payments

A non-refundable booking fee of 20% is required to reserve your surgical date. This amount is applied
toward the total cost of surgery.
The remaining balance is due in full, 3 weeks before your scheduled surgery.
Failure to submit full payment by the deadline will result in cancellation of your procedure.

Surgical Rescheduling

To reschedule a confirmed surgical date, an additional non-refundable 20% fee is required.

Surgical Cancellations

Cancellations for non-medical reasons within 3 weeks of surgery are non-refundable.

If your primary care physician determines you are not medically cleared for surgery, you will receive
a full refund, pending submission of a signed physician’s letter detailing the reason.

**This excludes any previous rescheduling or processing fees.

Introduction

Additional Charges

 

You may be responsible for additional charges, including but not limited to:
• Returned checks
• Medical records requests
• FMLA or disability paperwork
• Pathology fees
• Emergency hospital care during recovery
• Collection costs for unpaid balances
• Facility and anesthesia fees for any additional surgical care performed at our facility.

If legal action is required to collect any outstanding balance, you will be held responsible for all attorney
fees, collection agency costs, and any related legal expenses.
