Consultations require a non-refundable fee to secure an appointment at Parkins Plastic Surgery. The Surgical consultation fee is $100. The MedSpa consultation fee is $50. To schedule the MedSpa appointment, a valid credit card number will be held on file. *See cancellations below.
Cancellations made less than 48 hours in advance of your appointment are non-refundable. A credit card is required to be held on file and will be charged $50 for no-show or late cancellations.
All MedSpa services must be paid for at the time of service. All products are non-returnable/refundable.
If you are more than 10 minutes late or if required documentation is not completed prior to arrival, your appointment may be subject to be moved, cancelled, or partial treatment.
A non-refundable 20% booking fee is required to reserve your surgery date. This payment will be applied towards the cost of surgery. The remaining balance is due in full 3 weeks prior to the surgery date. If the balance is not paid, surgery is canceled.
Once booked, to move your surgical date, an additional non-refundable 20% fee is required.
If the patient cancels for non-medical reasons less than three weeks prior to surgery, no refunds are issued.
Patients whose primary care physician deems them not optimized for surgery will receive a full refund. A signed doctor’s note stating the patient is not suitable for surgery is required prior to funds being returned.
You are responsible for the payment of any additional charges or expenses that may be incurred, including, but not limited to: Returned Checks, Medical Records, FMLA documents, Pathology fees, hospital bills related to emergency care in the post-operative period, and any costs associated with collection on payments. In the event legal action should become necessary to collect an unpaid balance due, you will be fully responsible for our attorney fees, collection agency fees, and other related costs.
