Rhinoplasty

Improve Nasal Proportions, Symmetry, and Even Function at the Milwaukee and Madison Area’s Parkins Plastic Surgery

Since the nose assumes the role of the centerpiece of the face, its size, symmetry, and shape bear an undeniable impact on a person’s overall appearance. Even a subtle alteration in the nasal tip’s projection or rotation, or to the smoothness of the nasal bridge, can produce a seamless aesthetic appeal between the nose and the remaining facial features. Because the nose is so prominent, it is especially important that anyone seeking changes work with a plastic surgeon who is highly experienced in the procedure known as rhinoplasty. Madison and Milwaukee-area patients who want what is commonly called a “nose job” select Dr. Lucas Boehm for his knowledge and experience with advanced cosmetic surgery techniques performed to modify the shape or size of the nose—and even help to improve its function.

What's Included

Dr. Boehm’s approach to nasal surgery is one of preservation and establishing structural support. Throughout his training, Dr. Boehm has dedicated significant time to understanding the intricate anatomy of the nose and how alterations to this anatomy influence the external appearance of the nose both immediately and over time.

Traditional rhinoplasty operations were reductive in nature, focusing on making the nose smaller and more “refined.” This often led to a weakening of the supporting structures of the nose and deformity— sometimes years after the operation. Using newer, more modern rhinoplasty techniques and technology, Dr. Boehm is able to refine the external appearance of the nose without sacrificing its underlying support.

Personalized Rhinoplasty Consultations for Oconomowoc

More so than most other surgical procedures, aesthetic nasal surgery must be customized to the individual. A detailed analysis of not only the internal and external nose, but the entire face, must be performed to understand what structures require alteration to achieve a more aesthetically pleasing nose that remains functional and proportional to the existing facial structures. Dr. Boehm and his team utilize Vectra 3D imaging technology during your consultation process. This allows Dr. Boehm to simulate different changes to your nose and how various rhinoplasty techniques would alter your appearance. He believes this improves communication between himself and his patients, ensuring your aesthetic goals and his align.

VECTRA 3D Imaging System

Parkins Plastic Surgery is excited to announce our newest addition– the incredible VECTRA 3D Imaging Camera! VECTRA’s lifelike 3-D simulation will give you a complete preview of how your results are likely to appear on your body* – not an approximation on a model. With VECTRA, you can experiment with different surgical options and work with your surgeon to develop the best cosmetic plan for you. Have confidence in your decision with the VECTRA 3D Imaging System at Parkins Plastic Surgery!

The VECTRA Face Sculptor takes measurements of multiple facial features including the nose, chin, cheeks, and neck. Then, Dr. Boehm can sculpt the desired area to give a simulation on what your surgical results will look like*. The VECTRA software is a fantastic tool for patients to visualize surgical procedures such as a rhinoplasty, genioplasty (chin augmentation), facelift/necklift, and submental liposuction. A side-by-side comparison with a pre-surgical photo and the simulation helps patients feel more confident in their cosmetic decision. Patients love this feature because they can see their simulated results instantly.

What Makes a Good Rhinoplasty Candidate?

Not everyone is a good candidate for rhinoplasty surgery, and it is important to undergo a thorough evaluation by a qualified plastic surgeon to determine whether the procedure is right for you. Some general facts that may make someone a good candidate for rhinoplasty include:

Good General Health

Patients should be in good overall health and free from any medical conditions that could complicate surgery or increase the risk of complications.

Realistic Expectations

Patients should have realistic expectations for the outcome of the rhinoplasty procedure and be wiling to work closely with their surgeon to develop a treatment plan that aligns with their goals.

Non-Smoker

Smoking can increase the risk of complications and impair healing, so patients should be willing to quit smoking for at least four weeks before and after surgery.

Good Emotional Health

Patients should be emotionally stable and have a positive attitude toward the procedure, as well as a strong support system to help them through the post-rhinoplasty recovery process.

Specific Concerns

Patients should have specific concerns about the size, shape, or function of their nose that can be addressed through rhinoplasty or septoplasty surgery.

What Does a Rhinoplasty Involve?

Consultation

Dr. Boehm and his team will have a personalized consultation with you to go over your health history, physically examine you, and discuss your goals for nasal surgery. We will take photographs that will serve as baseline “before” image, and Dr. Boehm will review them with you. Vectra 3D imaging will be used to simulate various changes to your nose. After this, Dr. Boehm will explain his understanding of your cosmetic goals and make recommendations for meeting them.

Preoperative Appointment

About two weeks before your scheduled surgery date, you meet with Dr. Boehm and his team again. This meeting it to go over the proposed treatment plan, provide answers to any remaining questions, and highlight important preoperative and postoperative instructions.

Surgery

Surgery is performed on site at Parkins Plastic Surgery’s state-of-the-art AAAASF-certified operating suite.

Dr. Boehm performs rhinoplasty procedures with the patient under general anesthesia, which is administered by a board-certified anesthesiologist.

The surgery takes two to four hours, depending on what combination of procedures the patient is having done.

Dr. Boehm is dedicated to placing incisions with a goal of hiding any signs of surgery as much as possible. The incision across the columella is less than 5 mm wide and, when performed correctly, is imperceptible once healed.

Recovery

Patients recover from their general anesthesia in our Lake Country office, featuring a view of Silver Lake. Rhinoplasty operation is an outpatient procedure, which means you will head home to heal and recover under the care of a family member or loved one.

Dr. Boehm and his team will see you in the office the morning following your surgery to change dressings if needed, as well as review incision care and activity restrictions.

Patients should be prepared to take at least one week off from work following their rhinoplasty surgery. You will be on a cycle of medication and rest for the first several days.

One week after surgery, you return to the office. Your external splint (and internal splint, if present) is removed. Any visible sutures are removed. You will begin taping your nose nightly for three to six weeks to help reduce edema. You will begin irrigating your nose with saline spray two to three times daily. You will notice your breathing will improve dramatically with removal of your splints.

Three weeks after surgery, you return to clinic for a routine postoperative check. We will begin internal scar mitigation if necessary. Nightly nasal massage begins. Generalized bruising and swelling has improved significantly at this point.

Return to clinic at three months, six months, 12 months, and two years so Dr. Boehm can continue to monitor your results.

In general, keep in mind that swelling in the nose persists for much longer than in other areas of the body. A general rule of thumb is that patients lose one-third of nasal tip swelling by two months, the next third between three to nine months, and the final third between 10 to 12 months. Thus, the final result of your rhinoplasty is not evident for a full 12 months after surgery.

What Sets Dr. Boehm Apart for Rhinoplasty Surgery?

Rhinoplasty is often cited as one of the (if not the) most difficult operations a plastic surgeon performs. The procedure takes precise preoperative analysis, a thorough understanding of anatomy, and a meticulous attention to detail. Dr. Boehm’s passion for rhinoplasty surgery has taken him across the country and the world to learn the newest techniques from masters in the field. As a result of these experiences, Dr. Boehm employs a structural preservation approach to rhinoplasty, where he can alter the external appearance of the nose without sacrificing the underlying structural support. This allows for a beautiful, natural result that will persist for a lifetime.

What Procedures Complement a Rhinoplasty in the Milwaukee and Madison Area?

During your initial consultation, Dr. Boehm may suggest procedures that he believes will improve the balance of your face in conjunction with your rhinoplasty. This may include a chin augmentation or facial fat grafting. Many patients also seek out ear surgery to address overly prominent ears.
